Usually, when there is a need for root canal treatment due to trauma in the front teeth, i.e. a fracture or decay in the front teeth due to a bump or accident, the root canal treatment material used during the root canal treatment or the bleeding inside the tooth causes the tooth to turn purple-yellow over time. This is actually due to the structure of the inner surface of the tooth. In other words, the purple effect or yellow effect created by the bleeding inside the inner structure of the tooth that has somehow turned purple or yellow can be removed with a whitening method. Death of the Living Part of the Tooth (Pulp Necrosis)

It can cause necrosis of the pulp of the tooth due to bacteria inside the tooth or mechanical impact and fracture or chemical irritation. And tissue destruction products emerge. These tissue residues settle inside the small tubules inside the soft area called dentin inside the tooth and cause a color change in the tooth tissue. It is also possible to estimate how long the pulp has been dead according to the degree of discoloration. As the time increases, the discoloration becomes more severe and darker and even continues towards the root of the tooth.

Intrapulpal Bleeding

When there is bleeding inside the tooth as a result of trauma, this bleeding spreads inside the tooth and enters the tubules of the layer we call dentin, causing discoloration or bruising. If the pulp is lifeless, the discoloration is permanent and usually reaches all the way to the root. If the pulp heals, the tooth can return to its normal color. In cases where it does not heal, there is definitely a putrid layer inside the tooth and root canal treatment is necessary.

Dentin Hypercalcification

It is the increase of calcified structure in the inner part of the dentin. As a result of a collision and injury to the tooth, the pulp chamber root canals are filled with excessive dentin layer. Since the tooth is a transparent organ, the yellowness of the color of this area inside the tooth is reflected differently from other teeth and the tooth becomes dull and yellow.

Discoloration of the Inside of the Tooth with Age

With age, the enamel on the tooth thins and the dentin layer underneath becomes exposed, causing the tooth to take on a yellower, duller appearance. The tooth physiologically experiences a color change and takes on a darker color. Due to cracks and other changes on the enamel, this area changes color to an amber color in elderly patients.

Developmental Defects of the Tooth

It causes defects and discolorations of the tooth's enamel structure and the dentin formation beneath it. Enamel formation defects; defects in enamel formation are hypercalcific or hypoplastic. In other words, enamel hypercalcification is a specific area of ​​white or brown and is commonly found on the facial surface of the affected crown. Both brown spots and sometimes white spots occur. These are small worn areas. The element that distinguishes enamel hypoplasia, that is, white or brown discolorations inside the enamel, from enamel hypocalcification is the enamel being defective and porous. And this sometimes causes the tooth to change color by drying or taking in water or being stained by food residues.

Systemic Diseases

In systemic diseases that affect the general health of the person, teeth are discolored. Depending on the drugs used, especially tetracycline group antibiotics cause brownish yellowish discolorations in the internal structure of the tooth. In this type of discoloration, especially tetracycline discolorations, the discolorations appear in brown, yellow and even gray colors depending on the use of tetracycline and depending on time.

What are the Dentist-Related Reasons?

Tooth discoloration, especially the discoloration of the inner part of the tooth, can also occur during dentistry procedures. These include:

  • Remnants of pulp tissue
  • Drugs found in the canal
  • Root canal filling materials
  • Amalgams
  • Composites
  • The pins placed inside the tooth are posts.

In discolorations that occur following endodontic treatment, the remnants of the pulp tissue color the inside of the tooth and cause a permanent color change in the tooth.

In endo bleaching, after the elements that cause this odor and discoloration inside the tooth are removed in the canal and a good and ideal canal treatment is performed, medicine is placed inside the tooth with or without laser support. Sometimes, medicine is applied to dissolve the discoloration inside the tooth in one or two sessions. Sometimes heat sources are used. In cases where all teeth are generally colored, a special whitening method is used to lighten the color of both all teeth and the single tooth there.

Can Whitening Fail?

The bleaching process can cause re-coloring in rare cases that are resistant. However, there will be no color change in the bleaching process in a pulp chamber that is ideally closed, covered and cleaned. The color change will continue to be white for many years.

What are the materials used in whitening?

Usually hydrogen peroxide is used for this. Or sodium perborate is used. Sometimes materials developed for this process, produced entirely by companies, can be used.
