Broken Root Canal File (Root Canal Appliance) Situation During Root Canal Treatment: Don't Worry, There's a Solution
Although instrument (file) breakage inside a tooth during root canal treatment may be worrying for our patients, it is a known, encountered, and solvable situation in the field of endodontics (root canal treatment).
What is a canal file and why does it break?
During root canal treatment, extremely thin, flexible, needle-like instruments made of a special alloy (usually nickel-titanium) called "files" are used to clean and shape the hair-thin canals inside the roots of the tooth.
No matter how flexible and durable these devices are manufactured, they can sometimes separate (break) inside the canal:
• Anatomical Challenges: Your tooth roots may be excessively curved, narrow, or calcified.
• Metal Fatigue: High physical stress experienced by the instrument while working
in a very narrow canal. This is a physical stress created by the anatomical structure
of the tooth, rather than a dental error.
Could a Broken File Harm My Body or My Teeth?
This is one of the most frequently asked questions. The broken piece itself
is sterile and made of a biocompatible metal. It will not rust, cause poisoning, or enter the bloodstream in your body. It poses no problem for MRI imaging.
The real problem isn't the presence of the metal itself, but the fact that the broken piece is blocking part of the root canal. If bacteria remain trapped beneath this blockage, it can lead to an infection (inflammation) in the tooth later on. Therefore, the broken piece needs to be removed so that the area can be cleaned.
How is a fractured file treated?
Thanks to advancements in technology, the idea that "this tooth must be extracted" in case of a tooth fracture is a thing of the past. Our clinic follows these steps to resolve such challenging cases:
1. Obtaining a View with a Dental Operating Microscope: It is impossible to see a fragment deep within the root canal with the naked eye.
Using a Dental Operating Microscope, which can magnify the working area up to 40 times and has a powerful light source, the exact location, depth, and angle of the fractured fragment can be clearly determined.
2. Extraction with Ultrasonic Systems: After the fragment is clearly visualized under a microscope, special vibrating "ultrasonic tips" are used. These thin tips create micro-vibrations around the fractured file, loosening the fragment and allowing it to be safely expelled from the root canal. During this process, healthy tooth tissue is preserved to the maximum extent.
3. Bypass Technique: If the fractured piece is located at the very tip of the root, and removing
it could be risky and damage the tooth, the "bypass" technique is applied. Using special instruments, a thin path is created alongside the fractured piece, reaching the lower part of the root canal. The area is disinfected, and the piece is trapped (isolated) within the root canal filling, successfully completing the treatment.
4. Surgical Intervention (Apical Resection): In very rare cases where intervention through the root canal is not possible, a small surgical procedure is
performed to access the root tip from the outside and clean the infection and fractured fragments in that area. The tooth remains in the mouth.
Will I feel pain during treatment?
No. Just like with a standard filling or root canal treatment, the tooth and surrounding tissues are completely numbed with local anesthesia. You will not feel any pain during the procedure.
In summary;
Just because a root canal instrument breaks inside your tooth doesn't mean you'll lose that tooth. Thanks to advanced technologies like dental operating microscopes and modern endodontic techniques, these teeth can be saved and continue to serve you healthily for many years to come.
